Appointment request
This is a request, not a confirmed booking. The front desk will contact you during office hours to schedule.
Please don’t include symptoms, diagnoses, test results, medications or any other health details. Use a general reason for your visit only, and call the office for anything clinical or urgent.
This is a draft site with a non-functional form. Nothing was sent, saved or emailed, and no data left your browser. On the live site this would create a request for the front desk to call back during office hours.
On Riverside Drive in Savannah, near the Tennessee River. An interactive map will be embedded here before launch.
The entrance and the parking lot are both wheelchair accessible. Credit cards, debit cards and NFC mobile payments are accepted.
We work with most major insurance plans, including Medicare, and self-pay patients are welcome — call the office and we will check your coverage before your visit.
If this is a medical emergency, call 911. The office is open Monday to Thursday, 8:00 am to 4:30 pm, and Friday from 8:00 am to 11:30 am. Outside those hours, the phone line and this website form are not monitored unless you're a member — Membership includes a 24-hour call line and staff access outside office hours for individual and family emergencies.
